Visiting Health Care Staff

District Nurses:

Jan North (District Nurse Assessor)
Christopher Smith (District Nurse)
Jo Comer (District Nurse)
Karen Groom (District Nurse)
Gemma Williams (Health Care Assistant)

Jan heads up the team of district nurses. The team looks after those patients needing nursing care in their home. This will vary and as an example will include dressings, injections, phlebotomy, catheterisation, etc.

Jan's role includes the assessment of patients prior to discharge into the community.

Health Visitors:

Sara Bourne is the practice health visitor at our practice. 

Midwives: 

The midwives look after all our antenatal patients, from booking through to two weeks post delivery. They run 1 clinic a week in the surgery for antenatal care and liaise closely with the GPs and health visitors on the monitoring and management of patients. 

*Note: The midwives and the health visitors run a combined Parent craft evening for all prospective mums and dads. The programme runs over a course of 6 weeks and covers all aspects such as the birth, feeding, care of baby, milestones, immunisations, etc.

Community Psychiatric Nurse

Rob Clark is the Community Psychiatric Nurse attached to the practice. He liaises between the practice and the rest of the Community Psychiatry Team members. He works in the practice on Fridays. Patients are referred directly to him by the GP.

Counsellor:

The practice has a new counsellor, Hilary Chamberlain. Hilary works 12 hours a week at the practice. Patients are referred directly to her by the GP.

Dietician

The practice has a dietician who comes to the practice twice a month. The GP and practice nurse refers patients for dietary advice.

Carer Support Worker

The practice is fortunate to have a carer support worker based at the practice. Michael D’Agostino shares an office with the district nurses and is the patient and carer main contact for patients registered in the surgery area.
