Think before you repeat!

Request only the items required and ask us to align quantities of your medication so that you only need to put in one request a month.

Overstocking medication is dangerous as they go out of date quickly and your treatment may change.

Tell your doctor if you have stopped taking any of your medication as this will help reduce wastage in the NHS.  Your Doctor will not mind - The choice is yours.

If your medication is changed at review, you will be given a fresh request slip.  Discard all old slips and take all discontinued medication to your local pharmacy who will arrange safe disposal of all unwanted medication.

Tell your doctor if you are not happy with your medication or if it does not agree with you - he/she will be happy to prescribe an alternative where possible.
