Patient Identifiable Information
The practice holds information about all patients, which is private and sensitive, in both paper and electronic form. Unless an individual patient consents to the release of information then this information will not be made available to anyone other than staff of the practice who require access to it to perform their duties, or other medical professionals who need access to this information in order to provide a requested service to the individual patient.
Statistical And Anonymous Data
We may be required by authorised bodies, such as the Primary Care Trust, to provide
anonymous statistical data, which cannot be linked directly to any individual patient, to support the health commissioning process or as part of the national audit process. We will only provide such data if we are satisfied that it is totally anonymous and has been requested for a legitimate and properly approved purpose. |

CCTV
Please note that we operate a CCTV System in the practice. The data recorded via the system is stored for up to 14 days and may be used, where an incident has occurred, to assist the police in their inquiries.
